HVVC 2010 Policies

Coaches of USAV teams: Please fill these out and mail them to Rody Foco at 2109 Stone School Circle, Ann Arbor, MI 48108. We primarily need personal information and signatures. So, anything you don't know, leave blank and we'll fill it in. Do not pay anything, the club covers all of the fees.

Coaches, please download these, fill them out, and mail them to Beth Yoskovich, 10600 N. Platt Rd., Milan, MI 48160. You will also need to enclose copies of several forms of identification (see instructions on the form).
